Systemic Issues

A lack of flight hours and overworked maintainers have been reported to be the systemic root causes of aircraft crashes and other mishaps. These causes have been ongoing problems we are still trying to find solutions for.
In December of 2020, the National Commission on Military Aviation Safety published a report citing root causes to the thousands of military aviation mishaps.  The five-year study of this report recounts some of the leading causes which include inadequately trained aircrew and maintainers, aggressive operations tempo, overworked maintainers, and unpredictable budgets. Although this article focuses on the military, many of these root causes are systemic to almost every other aviation industry from commercial to contractual.  What can be done to mitigate the current problems and what can be done to prevent them from happening again?
